Referral & Care Coordination

At Agave Mental Wellness Institute, we value collaboration with providers, therapists, schools, and community partners to ensure patients receive compassionate, comprehensive mental health care. We welcome referrals for children, adolescents, and adults seeking psychiatric evaluations, medication management, therapy, and ongoing mental wellness support. Our team is committed to timely communication, coordinated care, and a patient-centered approach that promotes healing, growth, and long-term wellness. Self-referrals are also welcome, and we proudly accept most major insurance plans through our partnership with Headway. Please fill out the referral form below, and a member of our team will contact the patient within 48 business hours to assist with scheduling and next steps for care.
