How It Works
  • Contact us using the contact form or email us at contact@agavewellness.org. Please provide your information and any required documents so we can get started.

  • Click the Headway Sign-Up link below to verify your insurance benefits and securely add your payment information:

    Once completed, our team will review your information and contact you regarding scheduling and next steps.

  • You will receive an invitation to access the Omnipractice Patient Portal once your provider has added you to the system. Please enter your email to sign up, then use the verification code sent to your email to complete your registration. Be sure to use the same email address you provided to our office when creating your account through the link sent by Omnipractice.

    Please note: The Omnipractice login screen can be confusing and may appear to be intended for providers. However, patients should continue by using the invitation link sent to their email and signing in with the same email address used during registration.

    After logging in, please complete all required intake forms, questionnaires, and consent documents prior to your appointment.

  • Once you have completed your intake forms and insurance setup, you are ready to schedule your appointment. Our team will reach out with available appointment options, or you may be able to schedule directly through your patient portal if that option is available.

What You Should Do to Prepare

To help ensure a smooth and productive first appointment, please take a few minutes to gather the following information before your visit:

Your first appointment is an opportunity for your provider to get to know you and understand your concerns, history, and goals for treatment. You will review your intake information, discuss your symptoms in detail, and collaborate on an initial treatment plan tailored to your needs.

The session typically includes a comprehensive psychiatric evaluation, discussion of your medical and mental health history, and time to ask any questions you may have. If appropriate, your provider may also discuss medication options, therapy recommendations, or additional supports.

Please plan to attend your appointment in a quiet, private space to ensure confidentiality and minimize interruptions.

A complete list of all medications you are currently taking, including dosage and frequency

❋ Past Psychiatric Medications

A list of all psychiatric medications you have tried in the past, along with how effective they were and any side affects you experienced

❋ Medical & Mental Health History

Any relevant medical history, previous diagnoses, or prior treatment records (if available)

❋ Current Symptoms & Goals

A brief summary of your current symptoms, concerns, and what you hope to achieve from treatment
